Important: Please do not submit diagnoses, medication lists, or other detailed medical information through public website forms. Annie will collect any necessary care details privately after an initial call or tour.
What information we collect
When you submit a form on this site, we collect basic contact and inquiry details such as your name, phone number, email address, role, company or organization when applicable, interests, referral source, preferred response method, and any general message you choose to provide. Please do not submit diagnoses, medication lists, allergies, or other detailed medical information through public forms.
How we use information, cookies, and analytics.
We use the information you provide only to respond to your inquiry, arrange follow-up calls or tours, and keep track of our conversations. Inquiry information is stored in Wix CRM and Wix Forms. The site may use essential cookies and Wix Analytics to understand aggregate traffic, campaign sources, and form performance. Analytics campaign data is used for aggregate reporting and is not treated as consent to marketing.
Sharing and storage.
Your information is stored in Wix’s systems. Access is limited to Annie and administrators who need it to operate the website, manage inquiries, or comply with the law. We do not sell your information, use it for unrelated marketing, or export QR lead submissions to Google Sheets at launch. Wix and limited service providers may process information as needed to operate the site and communications.
Retention and your choices
We review QR leads monthly. If a person has not enrolled, has no open pipeline card, and the last recorded human contact was more than 12 months ago, we delete or anonymize the lead unless another legitimate requirement applies. Test submissions are deleted after verification. You may request access, correction, or deletion at any time by emailing goldenyearspings@gmail.com; we will respond to reasonable requests as soon as practicable.
